All applicants are evaluated using the same objective criteria in compliance with the Wisconsin Fair Housing Act (Wis. Stat. § 106.50) and federal fair housing laws. Background Questions

Answer all questions honestly. A "Yes" answer does not automatically disqualify you. Each application is reviewed individually considering all circumstances.

Criminal history is considered on an individualized basis per HUD guidance. Factors considered include the nature and severity of the offense, time elapsed, evidence of rehabilitation, and relevance to tenancy.

All JKW Properties units are smoke-free. Smoking is not permitted inside the unit or on the property.

Fair Credit Reporting Act (FCRA) Disclosure

In connection with your rental application, JKW Properties LLC will obtain a consumer report (including credit history) and/or an investigative consumer report (including background check, rental history, and personal references) from a consumer reporting agency.

You have the right under the FCRA to request additional information about the nature and scope of any investigative consumer report. You also have the right to request a copy of your consumer report from the reporting agency within 60 days of any adverse action.

If your application is denied in whole or in part based on information in a consumer report, you will be notified and provided with the name, address, and phone number of the consumer reporting agency that furnished the report.

Authorization to Obtain Consumer Reports

By checking the box below, I authorize JKW Properties LLC and its designated agents to obtain consumer reports and investigative consumer reports in connection with my rental application, including credit history, eviction records, criminal background, and rental references. I understand this authorization remains valid throughout my tenancy if my application is approved.

Authorization to Verify Information

By checking the box below, I authorize JKW Properties LLC to verify all information provided in this application, including contacting current and former landlords, employers, and personal references, and to confirm my identity using the provided government-issued ID.

Application Fee Acknowledgment

The non-refundable application fee is $40.00 per adult applicant (age 18+). This fee covers the cost of the credit and background check. Payment instructions will be provided after you submit this application. Your application will not be processed until the fee is received.

Applicant Certification

I certify under penalty of perjury that all information provided in this application is true, complete, and accurate to the best of my knowledge. I understand that any false, misleading, or incomplete information is grounds for rejection of this application or immediate termination of any resulting lease agreement.

I understand that submitting this application does not guarantee approval or entitle me to a rental unit. JKW Properties LLC reserves the right to reject any application that does not meet the stated qualification criteria, applied consistently to all applicants.
